From The Kansas City Star: The National Transportation Safety Board concluded that a bus driver’s “pedal misapplication” caused a Liberty (Mo.) district school bus to blast through an intersection in May 2005, killing two motorists, and injuring the bus driver and 48 students. The NTSB called for improved pedal design to help prevent bus drivers from mistakenly stepping on the accelerator while trying to brake. The bus driver has told investigators that she tried to apply the brakes but that nothing happened. The brake and accelerator pedals in the bus, a 2000 Thomas Built 84-passenger model, are about the same size and positioned two inches apart. Both are hinged at the bottom, so they depress in the same manner.
